Your mother sees a cardiologist, an endocrinologist, a neurologist and a family doctor. Each one prescribes. None of them has the full list. When she ends up in an emergency room, the first question is the one nobody can answer completely: what is she taking, and why.

MedReconcile is the answer to that question, on one page.

What You Record

Every medication with its dose, its form, and how it is actually taken. Who prescribed it and their specialty. Where it is filled. And the field that matters most and appears on no pill bottle: why it was started, written the way it was explained to you.

Allergies with the reaction and how severe it is, because a blank allergy section reads as unasked rather than as none. Conditions with the year they were diagnosed. Prescribers with their clinic and phone number.

The Page

Everything above exists to produce one printable PDF: the person, their allergies, every current medication with prescriber and reason, what was stopped and when, and the conditions behind all of it. You see exactly what the page contains before you create it, so you always know what you are handing over.

Print two. One for a wallet, one for the fridge. Share it with a sibling, save it to Files, or bring it to the next appointment instead of trying to remember.

A clinician often needs to know what was tried and when it ended, so stopped medications move to their own section rather than disappearing.

The record is for someone else by design. It asks for their name, their year of birth, their emergency contact, not yours.

Everything stays on the device. There is no account, no server, and no sync. Nobody else can read this list, including us.

What This Is Not

MedReconcile records what clinicians prescribed. It does not check for drug interactions, it does not advise on doses, and it does not replace a pharmacist, a doctor, or emergency care. Always confirm the list with the prescribing clinician or pharmacist before anyone acts on it.
