Six weeks after the last appointment, the neurologist asks how things have been. You know it has been hard. You cannot remember which week the shouting started, whether he slept better on the days he ate properly, or which of the things you tried actually worked.

MemoryLog is the answer to that question, and the handover for whoever takes the next shift.

What You Log

A moment as it happens: agitation, confusion, repetition, withdrawal, a calm hour. How hard it was, in your own reading. And the field that matters most six weeks later, what helped. Closing the curtains before dusk. The 1962 records. Walking around the block. You will not remember which one it was.

Sleep, appetite and fluids by day. Medication with the dose, the time, and who gave it, because with several people helping that is the question every handover turns on. Incidents, with what helped and whether a clinician was told.

The Time of Day Pattern

The app counts difficult moments by hour and shows you where they cluster. Late afternoon clustering is what carers and clinicians call sundowning, and seeing it on a chart is what turns "the evenings are bad" into something a clinician can work with. It stays quiet until there are enough entries to mean something, rather than inventing a pattern from two.

The Handover Pdf

One document: the pattern, what has helped, sleep and appetite by day, the behaviour log, medication with who gave it, and every incident. Print one for the fridge so the next carer starts informed, and bring one to the neurology appointment.
